Telehealth disclaimer
What BridgeWell does, what CareValidate does, and what online care can and cannot do for you.
1. Clinical care and provider review
Clinical intake, evaluation, provider review, prescribing decisions, pharmacy fulfillment, and clinical follow-up are delivered by licensed clinicians through CareValidate's secure clinical environment. Treatment decisions are made by these licensed clinicians, who are authorized to practice in your state.
2. CareValidate's role
CareValidate handles the clinical surface — intake, provider review, prescribing decisions, and medical records. CareValidate is the system of record for clinical content. Clinical messages and sensitive medical information belong in CareValidate's secure clinical portal.
3. BridgeWell's role
BridgeWell coordinates the non-clinical journey — account access, billing status, reminders, movement, nutrition, coaching, progress tracking, and long-term program support. BridgeWell does not prescribe and does not provide medical advice.
4. No guarantee of treatment
Payment of the BridgeWell enrollment fee does not guarantee a prescription, treatment, or clinical approval. A CareValidate clinician may decline to prescribe at any point and may discontinue therapy if your health status changes. Specific medication options offered (including any compounded formulations) are explained at the time of clinical review.
5. Emergency disclaimer
BridgeWell is not an emergency service. Telehealth visits and asynchronous messaging cannot replace in-person care for every situation.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Do not use the BridgeWell portal or messaging surfaces to report a medical emergency — they are reviewed during business hours and are not monitored continuously.
For mental-health crises (United States), call or text the 988 Suicide & Crisis Lifeline, or contact your local crisis service. Mental-health emergencies are outside the scope of BridgeWell's metabolic-health program.
6. Privacy and clinical information
Sensitive clinical information belongs in CareValidate's secure clinical environment. The BridgeWell Privacy Policy governs how non-clinical information you provide to BridgeWell is stored and shared. Clinical records held by CareValidate are governed by CareValidate's privacy practices.
Information needed to coordinate your care is shared between BridgeWell and CareValidate; it is not sold to third parties.
7. Member responsibilities
Your intake and ongoing check-ins are the basis for clinical decisions. You agree to provide accurate information and to promptly report side effects, new diagnoses, hospitalizations, pregnancies, and any new medications during your time on the program.
Telehealth treatment is limited to states where the CareValidate clinician is licensed. Notify your care team before relocating or for extended travel — telehealth services may pause or transfer until you are seen by an in-state provider.
Acceptance
By completing the consent step of intake, you confirm that you have read this disclaimer, understand the limits of telehealth, and agree to the Terms of Service.
